Add the following under “Order Notes” on checkout.

1. First and Last name or name as you wish (This will replace the name “SALLY SMITH”)
2. Your stylist code (This will replace the code “PS100”)
3. Phone number and your email. If you wish not to have either or both, just indicate so.

Note: Select Local Pickup as a shipping option so you do not have to pay for shipping! The final design will be emailed to you once ready in printable format.
